7/16/2023 EnchantaVaughn Approved Behavior: QMHP-A greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-A inquired about her afternoon. Ms. Broadnax verbalized to QMHP-A that her afternoon was going well. QMHP-A informed Ms. Broadnax that this session was going to focus on developing coping skills to assist her with managing her frustrations. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHP-A began the session by inquiring about Ms. Broadnax’s current thoughts, feelings and emotions. QMHP-A informed the Ms. Broadnax that the session topic would concentrate on reviewing coping and calming methods to assist in managing anxiety, depression and stressors. QMHP-A directed the Ms. Broadnax to guide her through how to use coping and calming skills. QMHP-A followed the Ms. Broadnax’s lead as the Ms. Broadnax demonstrated coping and calming skills that were used to reduce overall anxiety and manage any anxiety or depressive symptoms. QMHP-A gave redirection, praise, and encouragement as needed. QMHP-A provided the Ms. Broadnax with resource material about progressive muscle relaxation and other calming strategies. QMHP-A inquired if the Ms. Broadnax had any questions, comments, and/or concerns. QMHP-A and Ms. Broadnax recapped the session. Response: Ms. Broadnax appeared to be receptive to QMHP-A’s interventions and services that were provided to her during the session. Ms. Broadnax showed the QMHP-A the relaxation skills that consisted of progressive muscle relaxation, guided imagery, and paced breathing. Ms. Broadnax used music from her cellphone to assist her in the techniques. Ms. Broadnax displayed a focused and communicative manner throughout the session evident by her attentive and open character. Ms. Broadnax actively participated in the session evident by her positive interactions and communications. Ms. Broadnax was receptive of the QMHP-A’s interventions evident by her responses and questions.
